Community Crossroads is a nonprofit Area Agency for individuals with Developmental Disabilities, Acquired Brain Disorders, and Children with Chronic Health Conditions. They also provide Representative Payee Services and Independent Service Coordination for adults served through the Choices for Independence Waiver.
Community Crossroads’ offerings include services for individuals with Developmental Disabilities: Birth to age 3 Early, Supports, & Services. 3 years to 21 years old: Family Support Services, Respite, & Information & Referral, & In Home Support Services. 21 Years Old Services
Funded through NH Medicaid under the Developmental Disabilities or Acquired Brain Disorder Waiver; including Supported Employment, Community Participation Services, & Residential /Personal Care Assistance.
