Community Partners offers comprehensive mental health care, individualized services for persons with a developmental disability, or family education and support services.
Community Partners is the designated Community Mental Health Center, Developmental Services Provider and Family Support Center for Strafford County. We offer a wide variety of behavioral health and developmental services across the lifespan to individuals and families.
We connect our clients and their families to opportunities and possibilities for full participation in their communities.
We serve those who experience emotional distress, mental illnesses, substance use disorders, developmental disabilities, chronic health needs, acquired brain disorder, as well as those who are in need of information and referral to access long-term supports and services.
We also are a ServiceLink provider.
ServiceLink is a program of the NH Department of Health and Human Services, who contracts with Community Partners to help individuals in Strafford and Rockingham Counties access and make connections to:
- Long term services and supports
- Family caregiver information and supports
- Understand and access Medicare and Medicaid options
We also offer the Choices for Independence (CFI) home and community-based program funded by NH Medicaid, providing a wide range of services designed to help eligible individuals remain in their home and community.
