DHHS, Family Centered Early Supports and Services (FCESS)

Family Centered Early Supports and Services supports the family’s and other caregiver’s ability to care for a child (age 0-3) with delays or disabilities to address their individual needs

How can they help your child and family?

  1. Develop an Individual Family Support Plan (IFSP) including a plan for service delivery
  2. Assist with coordinating services across agency lines
  3. Strategies are identified for implementation in the natural environment and across daily routines and interactions
  4. Providers work with families utilizing a coaching method
  5. Assist with transitioning to preschool special education program, if appropriate or a community setting

Family-centered early supports and services are provided within the child’s home or where a child spends time. FCESS are provided in all communities within NH and include any of the supports or services:

  • family support
  • education & counseling
  • vision services
  • hearing services
  • health & nursing services
  • medical
  • diagnostic & evaluation services
  • nutrition counseling & assessment
  • occupation therapy
  • physical therapy
  • special equipment
  • special instruction
  • speech & language therapy
  • transportation services
  • service coordination

Monday – Friday
8:00 a.m. – 4:30 p.m.
