Families in Transition – New Horizons

Families in Transition – New Horizons provides shelter and housing to individuals and families, along with food services (food pantry and soup kitchen) and supportive services (substance use treatment and case management).

The agency provides shelters for families and individual adults, with the goal to support a successful move into permanent housing. Our programming has been highly successful, enabling the vast majority of its participants to achieve stability and attain/maintain permanent housing. Shelters are located in Manchester and Wolfeboro. Case Management services are provided to offer a strengths base approach to achieve goals.

Families and individuals can utilize the pantry one time per month and receive pre-packed groceries based on the number of individuals residing in the household. Choices of fresh fruits, vegetables, meats and bakery items are also available based on family size. The bread line is open following the pantry distribution times. Clients using the pantry and others can come and get bakery and perishable items. Pantry clients may access the bread line as frequently as necessary.

The Willows Substance Use Treatment Center recognizes how substance use can destroy the lives of many, and is all too often a root cause of homelessness. Families in Transition – New Horizons-New Horizons offers specialized treatment programs for those in the community who struggle with substance use and mental health disorders.
