Provides programs and services to individuals 3 years and up with either a Developmental Disability or an Acquired Brain Disorder.
Family Support offers guidance and assistance to eligible families and members with members at home experiencing a disability.
Supports include:
- In-Home Supports,
- Respite Care,
- Environmental Modifications,
- Family Support Council support with advocacy, education, social activities and financial help to meet needs and enhance the quality of life of the individual with a disability.
Resource Coordination works directly with individuals and families to help them navigate benefits, resources and make well-informed choices at all stages of life. We also help with school-to-community transition planning. Self-Directed services offers the option to design, manage and direct your own support services.
We also offer Benefits Consultation, Vocational and Residential placement.
