MDS Children’s Service Coordination (CSC) provides resources to families who have a child (birth to 21) with a developmental disability or chronic health condition. The services provided are initiated by each family and are unique to their specific needs. Children Service Coordinators/Case Managers assist families in accessing services for their children. We educate and empower families to identify, access and utilize available resources in ways that are most helpful to them.
Children’s Service Coordinators (CSC) provide:
- information and referral to community services
- Early Supports and Services (ESS)
- assistance in planning for transitions
- access to training
- assistance with future planning
- use of lending library
- information about acquiring benefits
- parent-to-parent matches
- referral to appropriate support programs
- assistance to families with the special education process
Early Supports and Services
Early Supports and Services (ESS) are home-based, family-centered services and therapies for children under the age of three who have a delay of at least 33% in any area of development, an established condition, or who are at risk for developing a delay. These services are provided at no cost to eligible families.
To find out about eligibility for services, call 603-352-1304 or email the MDS Intake Coordinator who can inform you and lead you through the eligibility process, and then also help in setting up services once eligibility is determined.
